Thermotherapy
In general, proper thermotherapy to an injured area raises skin temperature and increases the amount of blood flow to the area. Heat can also be used to relieve joint stiffness, pain, muscle spasm, and inflammation, and to increase the extensibility of collagen tissues. Temperatures should not exceed 116° and a treatment session should never exceed 30 minutes. Additional cautions in the use of heat include:
1. Never apply heat immediately after an injury.
2. Never use heat when there is loss of sensation or decreased arterial circulation.
3. Never apply heat directly to the eyes, genitals, or the abdomen of a pregnant woman.
4. Never fall asleep while applying heat or apply heat over Ben Gay® or other topical heat ointments.
Heat can be safely applied through the use of moist heat and commercial packs as well as whirlpool and paraffin baths. You can use moist heat at home by soaking a towel in hot water and allowing it to drain for several seconds before applying to an injured area, which is already covered by four to six layers of toweling. The moist towel should not directly contact the skin.
Electrotherapy
The use of electrotherapy as a form of heat should be performed only by a physician, physical therapist, or licensed athletic trainer.
Massage
The use of massage to manipulate soft tissue is a helpful adjunct to heat and cold. Stroking, kneading, friction, percussion, and rapid shaking are some of the more common techniques used to increase heat, improve blood flow to the injured area, remove metabolites such as lactic acid, overcome edema, improve circulation and the venous return of blood to the heart, and aid relaxation.
