Generally a disease of childhood, rubella is often referred to as "German measles" because it was closely studied in Germany in the 19th century. The rubella virus infects the outer layer of cells of the upper respiratory tract but may replicate in the cervical lymph nodes. The virus appears in the blood about a week after infection and persists for an additional two weeks. A characteristic skin rash develops when the antibody appears in the blood. Rubella infection may have serious effects if contracted during early pregnancy. Infection during the first 10 weeks of pregnancy can lead to birth defects in the fetus.
Children are generally given the combined measles, mumps, and rubella vaccine at 15 months. Adults may be immunized at any time, although the majority of adults are already immune, even if they have no history of the disease. If a woman is pregnant and there are unimmunized children in the household, those children should be vaccinated, as they are the most likely rubella carriers.
