GENERAL TREATMENT MODALITIES: CRYOTHERAPY

The application of cold (cryotherapy) to the skin for 20 minutes or less at a minimum temperature of 50° F causes the constriction of vessels and reduces the flow of blood to the injured area. When cold is applied for longer than 20 minutes, an intermittent period of vasodilation occurs for 4 to 6 minutes. This prevents tissue damage from too much exposure to cold. At this point, cold is no longer effective. It also reduces muscle spasm, swelling, and pain; slows metabolic rate; and increases collagen inelasticity and joint stiffness. Cold is somewhat more penetrating than heat, and the effects last longer.
Cold applications should be used immediately after an injury and continued for several days until swelling subsides. Cold can be applied intermittently for 20 minutes every 1-1/2 waking hours in combination with compression, elevation, and rest. The longer the cold is applied, the deeper the cooling. Ice massage can be used on a small body area by freezing water in a plastic-foam cup to form a cylinder of ice. After removing one or two inches of the plastic foam at the top of the cup, the cup portion can be used as a handle, and the ice can be rubbed over the skin in overlapping circles for 5 to 10 minutes to produce cold, burning, aching, and numbness in the area. Ice packs can be made by placing flaked or crushed ice in a wet towel or self-sealing plastic bag. Unless ice massage is being used, ice should not come in direct contact with the skin.
